Free 12-hour shift schedule templates for 24/7 teams

Seven rotations that fill themselves in: type a start date and each crew’s days and nights appear, with coverage counts and the overtime each workweek creates. Every template shows a worked example before you download it.

Quick answer

  • Most 24/7 teams start with the Pitman 2-2-3 template: four crews, 12-hour shifts, 42 hours a week on average and every other weekend off.
  • Every rotation that averages 42 hours creates overtime in its long weeks. The templates calculate it for each workweek so you can budget before you commit.
  • All 7 templates are free Excel files with a printable PDF. Change the start date and the whole calendar updates; they also open in Google Sheets.

A 12-hour shift schedule template lays out which crew works days, which works nights and who is off, so a plant, hospital unit, 911 center or security team is covered around the clock. This page is for US operations managers, nurse managers and owners who run 24/7 or 24/5 coverage. Below are seven free templates: the four 12-hour rotations people search for most (Pitman 2-2-3, DuPont, 4-on-4-off and 7-on-7-off), a two-week nurse schedule for three 12s a week, a shift pattern template that builds any rotation you type, and a rotating 8-hour three-shift schedule for teams that are not on 12s. Each Excel file fills in the calendar from a start date and a pattern, counts how many crews are on each shift, and shows each crew’s hours and FLSA overtime for every workweek.

Which 12-hour rotating schedule fits your team?

All four-crew 12-hour rotations average 42 hours a week, because four crews share 168 hours of coverage per position. What differs is how those hours fall into weeks, how long people stay on, and how long their breaks are.

Overtime figures assume a 12-hour paid shift (8 hours for the last row), a Monday-to-Sunday workweek and the crew offsets in our templates, under the federal 40-hour rule only. Your numbers change with your workweek start, your unpaid meal breaks and state law, which is why every template recalculates them.

Excel: four crews over 28 days, filled from a start date, with day and night coverage counts, weekly hours per crew and FLSA overtime. PDF: the 28-day pattern to post on the wall.

The Pitman schedule repeats every 14 days: 2 on, 2 off, 3 on, 2 off, 2 on, 3 off. Crews A and B split the day shift and crews C and D the nights, so nobody rotates between days and nights. Each person works 7 shifts every two weeks, 84 hours in all, and has every other weekend off. In the template the crews are offset so each week is 36 or 48 hours, which means 8 hours of overtime every two weeks. Our Pitman schedule guide explains the pattern in more depth.

Excel: the full 28-day DuPont cycle for four crews with coverage checks, weekly hours and overtime. PDF: the cycle in one page, landscape.

Each crew works 4 nights, takes 3 days off, works 3 days, 1 off, 3 nights, 3 off, 4 days, then gets 7 days off in a row. The seven-day break is the selling point. The price is a 72-hour week in every cycle and a switch from day shifts to night shifts with only one day off between them. As laid out, each crew’s weeks run 48, 72, 48 and 0 hours, which is 48 overtime hours every four weeks under the 40-hour rule. Read our DuPont schedule guide before you propose it to a crew; this 12-hour shift schedule template shows them every day of the cycle at once.

Excel: eight weeks (56 days) for four crews, so you can see the pattern drift through the calendar, with weekly hours and overtime. PDF: two 28-day pages.

Four shifts on, four days off, repeat. It is easy to explain and easy to swap within, but the 8-day cycle does not line up with a 7-day week. Days off move through the calendar, so weekends off come and go, and a crew works 48 hours in some weeks and 36 in others. Over eight weeks that is 32 hours of overtime per crew. The Excel file shows exactly which weeks are long for each crew, so you can plan vacations into them.

Excel: 20 staff over two weeks with D, N or PTO picked from a list, shifts and hours per week, overtime, and rows for RN and CNA days and nights needed versus scheduled. Short shifts turn red.

Many nurses work three 12-hour shifts a week rather than a fixed rotation, and the manager builds the schedule around requests. This template is for that: each nurse’s row counts shifts and hours per workweek, and the coverage rows at the bottom show any day or night that is short of your minimum. In the example every RN works three 12s a week and days are covered, but Sunday nights show 0, which is the gap the sheet turns red so you can book a per diem nurse before the schedule is posted. Three 12s is 36 hours, so a fourth shift in the same workweek means 8 hours of overtime under the 40-hour rule. If your hospital uses the 8-and-80 system, compare hours with the 14-day total and the 8-hour daily limit instead; under it, each 12-hour shift includes 4 overtime hours. See our healthcare scheduling software guide when the spreadsheet gets too big for one unit.

Excel: four crews over 28 days with coverage counts, weekly hours and overtime. Change the offsets to test a different changeover day. PDF: the cycle on one page.

A full week on and a full week off suits sites where people travel far to work. Where the workweek falls matters more here than in any other rotation. If the workweek starts on the changeover day, a week on is 84 hours and 44 of them are overtime for a non-exempt employee. In the template, crews change over on Friday while the workweek starts Monday, so each seven-shift block splits into a 48-hour week and a 36-hour week: 8 overtime hours every two weeks. Decide this before the first block starts, not afterward. Seven 12-hour shifts in a row is also a fatigue question, so set rules for rest and breaks.

6. Shift pattern builder (any rotation)

Excel: six crew rows, each with its own pattern string, start offset and 56 days of calendar, plus day, evening and night coverage, weekly hours and overtime. Hours per shift is one cell.

Type a pattern using D for day, E for evening, N for night and a dash for a day off, of any length: DDD--- for 3 on, 3 off, or a 28-letter string for a custom rotation. The offset says where in the pattern each crew starts. Then read the coverage rows: every day should show exactly the number of crews you need on each shift. If a day shows 0 or 2, adjust an offset. This shift pattern template is the quickest way to test an idea from the crew before you commit to it.

Excel: three crews over a 21-day cycle with day, evening and night coverage and weekly hours. Change the offsets to rotate backward or the hours per shift for 8.5-hour shifts.

Not every rotating shift schedule template needs 12-hour shifts. Here each crew spends a week on days, a week on evenings and a week on nights, then starts again, with weekends off and 40 hours a week. The rotation runs forward (days, then evenings, then nights), and the weekend between blocks gives people time to adjust their sleep. If you add Saturday overtime, each extra shift is 8 hours over 40.

How overtime works on 12-hour shifts

Federal law counts overtime by the workweek: a fixed, recurring 168-hour period that can start on any day and at any hour. Hours cannot be averaged across two weeks. That is why rotations that average 42 hours still owe overtime in their long weeks, and why the same rotation can cost more or less depending on where the workweek starts.

The Pitman pattern shows it clearly. Here is the same 14-day cycle measured against different workweek start days:

Where the workweek starts in the cycleWeek 1 hoursWeek 2 hoursOvertime per 2 weeks
First day of the first 2-shift block602420
Second day of that block48368
First day off after that block36488

The difference is 12 overtime hours per person every two weeks, and on a 7-on, 7-off rotation it is 36 hours. Choose the alignment when you set up the rotation. Federal rules let you set different workweeks for different groups of employees, but a change later has to be intended as permanent and not designed to evade overtime. To check a single employee’s pay for a week, use our overtime calculator.

Two more costs to plan for. Paid rest breaks count as hours worked, so they never reduce the total. And a state with daily overtime changes the math completely: California pays 1.5 times the regular rate after 8 hours in a workday and double time after 12. A properly adopted alternative workweek schedule can raise the daily threshold to as much as 10 hours (12 in health care), but hours over 12 are still double time.

How to use a 12-hour shift schedule template

  1. Count your positions. For each post that must be covered 24/7, you need four people on a four-crew rotation, plus relief for vacations and sick days.
  2. Pick the rotation with the crew. Share the comparison table and the PDFs. People care most about weekends off, the longest run of shifts and whether they rotate between days and nights.
  3. Set the start date to the first day of your workweek and check the overtime column. Move the crew offsets to see which alignment works best, then fix it.
  4. Check coverage. Every day should show the number of crews you need on days and on nights. A 0 is a gap; a 2 is wasted labor.
  5. Add names and post it. List each crew’s members, print the PDF, and post it at least as far ahead as your policy or local scheduling law requires.
  6. Track real hours separately. The schedule is the plan; swaps, call-offs and late punches change the actual hours. Keep timesheets or a time clock for pay.

Crews that live on a rotation trade shifts constantly. If you are tracking swaps on paper, a scheduling app that handles shift trades and overtime alerts will save more time than any template. Our 2-2-3 schedule guide covers how teams roll out a new rotation.

These are general principles, checked on federal and state agency sites. They change, and states and cities add their own, so confirm with your state labor department.

  • Overtime is weekly under federal law. The FLSA requires at least 1.5 times the regular rate for hours over 40 in a workweek for non-exempt employees, and the Department of Labor says averaging hours over two or more weeks is not permitted.
  • Hospitals and residential care have an option. Under the 8-and-80 system, with an agreement made before the work is done, overtime is due for hours over 8 in a workday and over 80 in a fixed 14-day period.
  • Some states have daily overtime. California requires 1.5 times the regular rate after 8 hours in a workday and double time after 12, and overtime on the seventh consecutive day of work in a workweek.
  • Breaks depend on your state. Federal law does not require meal or rest breaks, but short breaks you do give are paid. California, for example, requires a 30-minute meal period before the end of the fifth hour, a second one for work over 10 hours, and a paid 10-minute rest break per four hours worked or major fraction.
  • On-call time can be work time. The DOL says an employee required to stay on call at the employer’s premises is working; on call at home generally is not, unless the restrictions are heavy.
  • Keep the schedules. The FLSA requires payroll records for at least three years, and lists work and time schedules among the records kept for two years. Keep the hours worked each day and each workweek.
  • Scheduling laws may require notice. Oregon requires covered large retail, hospitality and food service employers to post schedules 14 days ahead, and Seattle requires covered retail and food service employers to post 14 days ahead and pay time-and-a-half for close-to-open shifts less than 10 hours apart.

Frequently asked questions

What is the most common 12-hour shift schedule?

The Pitman, or 2-2-3, schedule is the most widely used 12-hour rotation for 24/7 coverage. Four crews work 2 days, take 2 off, work 3, take 2 off, work 2 and take 3 off over 14 days. Each person works 84 hours every two weeks, an average of 42 hours a week, and gets every other weekend off.

Do 12-hour shifts mean overtime?

Not under federal law by themselves. The FLSA requires overtime only for hours over 40 in a workweek, so three 12-hour shifts (36 hours) owe none. Rotations that average 42 hours do create overtime in their longer weeks. California and a few other states also count daily overtime, which makes every 12-hour shift costlier there.

How many employees do I need for 24/7 coverage with 12-hour shifts?

For one position covered around the clock, you need four people, one per crew: two crews cover days and nights while the other two are off. Multiply by the number of people each shift needs. Add a float or part-time pool for vacations and sick days, because every absence on a four-crew rotation is a shift someone must pick up.

Can I change the workweek start day to reduce overtime?

You can choose any fixed workweek start, and the choice changes how a 12-hour rotation’s hours fall into weeks. Under federal rules you may change it later only if the change is meant to be permanent and is not designed to evade overtime. Pick the start day when you set up the rotation, then keep it.

What is the difference between the Pitman and DuPont schedules?

Both use four crews and average 42 hours a week. Pitman repeats every 14 days with short stretches of two or three shifts and keeps people on days or nights. DuPont repeats every 28 days, rotates crews between days and nights, includes a 72-hour week, and ends each cycle with seven days off in a row.

Do nurses on 12-hour shifts get overtime?

Hospitals and residential care facilities may use the 8-and-80 system if they agree on it with employees in advance: overtime is then paid for hours over 8 in a day and over 80 in a fixed 14-day period, instead of over 40 in a week. Otherwise the normal 40-hour workweek rule applies. State law can add more.
